Any information that you give to me will be held safely and securely either as paper records stored in a lockable cabinet in a lockable room and or securely on my home PC or laptop.
I will not share your information with anyone without your permission. Examples include liaising with GPs, consultants and other health professionals.
I will not use your personal information for marketing purposes and I will never give your personal details to a third party unless agreed by yourself. See above.
By signing the consent form you are agreeing to me holding your data and contacting you by phone, text or email. You also agree for anonymous data to be used in ongoing clinical audits.
